INSTRUCTIONS
Please read the following instructions carefully before filling up the Application Form
1. Eligibility
(i) Father of candidate must be a domicile of UP.
(ii) Date of Birth Application are invited as per the given format from boys born between
dates given as under for the academic year 2015-16, for appearing in UP Sainik School
Entrance Examination to be held on 18 January 2015.:-
(a) For Class VII : 02 July 2003 to 01 January 2005 (both dates inclusive)
(b) For Class IX : 02 July 2001 to 01 January 2003 (both dates inclusive)
(iii) Must have passed class VI and VIII for admission in class VII and IX respectively by 20
June 2015 from a recognized school.
2. Reservation: Out of total vacancies, 27 % of the seats are reserved for OBC, 21 % for SC and 01%
for ST categories.
3. Scheme of Written Examination: Admission to the school is made through a competitive
examination. The subjects of examination are First Paper: Mathematics, Second Paper: Language
ability (Hindi & English) Test, General knowledge and intelligence test. The school provides
admission in class VII and IX only.
4. Examination Centres Agra, Allahabad, Bareilly, Faizabad, Gorakhpur, Jhansi, Meerut,
Varanasi and Lucknow.
5. Availability of Application Form: Format of Application form with instructions will be available
from 10 Sep 2014 to 10 Nov 2014 on school website www.upsainikschool.org.
6. Last Date for submission of Application Form Application completed in all respects alongwith
admit card, Identification Form, self address envelop with Rs.25/- stamp affixed and Demand Draft
of Rs 500/- drawn in favour of Principal, UP Sainik School, Lucknow payable at Lucknow towards
Registration Fee must reach the 'Principal, UP Sainik School, Sarojininagar, Lucknow, UP 2260008
on or before 10 Nov 2014. Incomplete application will be rejected and no communication will be
entertained in this respect. REGISTRATION FEE ONCE PAID IS NOT REFUNDABLE UNDER
ANY CIRCUMSTANCES. Application will be accepted by registered post/speed post only. The
school does not take any responsibility for postal delay/lapse in the delivery of these items.
7. Date of Entrance Examination 18 January 2015 (Sunday)
8. Tentative dates of Interview Between 2nd March to 07 March 2015
9. School Fee Rs. 30000/- per annum (Rs. 15000/- per term). Scholarship is given on merit basis.
Fee may be increased as per existing rules.
10. One box each to be left blank after First Name, Middle Name and Last Name in the application form
at serial number 4 to 6 and in Admit Card at serial no. 5.
11. Parents are advised to keep a photocopy of Demand Draft for further reference purpose.
12. The following documents/certificates to be submitted at the time of admission:
a. Transfer Certificate from the school last attended duly counter signed by District Inspector
of school.
b. Domicile certificate of father issued by the District Magistrate or authorised by DM in
original.
c. Caste certificate (SC/ST/OBC) from competent authority in original.
d. Agreement bond on Rs.50/- Special Adhesive Stamp.
e. Affidavit on Rs.10/- non judicial stamp regarding ragging.
f. Birth Certificate from Nagar Nigam/Nagar Palika/Jila Panchayat Adhikari in original.
